A. The application shall be a written statement, upon forms provided by the license officer, and shall include an affidavit, to be sworn by the applicant.

B. A duplicate license or special permit shall be issued by the license officer to replace any license previously issued, which has been lost, stolen, defaced, or destroyed, without any willful conduct on the part of the licensee.

C. The license officer shall, upon disapproving any application submitted under the provisions of this title, refund to the applicant all fees paid in advance, provided the applicant is not otherwise indebted to the city. [Ord. 01-17-2023C § 1. Code 2000 § 16-1-11].
