5.05.130 License form.
This section is included in your selections.
Each license issued hereunder shall state upon its face the following:
A. The name of the licensee and any other name under which such business is to be conducted.
B. The kind of each business so licensed.
C. The address of each business so licensed.
D. The amount of the license fee therefor.
E. The dates of issuance and expiration thereof.
F. Such information as the license officer shall determine. [Code 2000 § 16-1-13].
